Forms Needed to Complete a
Master's Degree in Instructional Technology Degree

 Advancement to Candidacy Form 

1. Obtain an Advancement to Candidacy Form from your academic advisor or from Cindy Wright in Zook 207 (College of Education [COE] main offices-second floor).
    

2. Complete the name and address portion as per the directions.
    

3. The Degree code is:

510001 - MA – Educational Foundations, Instructional Technology 

4. Current GPA – use the Online Information Center on the University of Akron home page (www.uakron.edu) to search your records.
    

5. Compare the courses listed on your academic record with the courses listed on the Program Course Description (PCD) prepared with your advisor at the beginning of your program. Request a current copy from your advisor if necessary. Note any discrepancies and share that information with your advisor.
    

6. Sign and return the Advancement to Candidacy form to your advisor at least one week before the forms are due.

Application to Graduate Form

The University of Akron conducts three commencement ceremonies a year. These occur at the end of fall, spring and summer semesters. A student must submit a graduation application to the Office of the University Registrar no later than the deadline identified in the following schedule:
 

To obtain the application to graduate form, go to the Registrar’s website: http://www3.uakron.edu/registrar . Complete required fields. Print the form, sign and mail the form to:
